I HAVE been experiencing great difficulty in making telephone calls to Pakistan these days. The quality of sound is very poor and hardly audible.
Words are broken and sentences remain incomplete. More often than not the connection breaks in the middle of the call. Interestingly, the recipients report that calls from abroad are indicated as originating from some local telephone numbers. I have not experienced such problems while making calls anywhere else in the world.
It is not a a one-time affair either. It happens all the time. Imagine the frustration of anyone trying to make urgent business or emergency calls to Pakistan. It is unacceptable in modern day and age and needs to be corrected as soon as possible.

I have experienced the same from Norway. But I do not expect anything less from the NRO governing system. Why should telecom be spared when they have/are eating everything else!?

I have experienced the same problem, while I was using the upstart telephone companies from USA, Europe, Australia..., which used calling cards or a similar subscription service. I ditched them, I have been using the standard national or international carriers since then, and never had a problem. With these smaller mushroom companies, the biggest problem is on Eid days, as these companies have limited infrastructure and load a lots of customers. Told ya earlier - CHEAP THINGS ARE GOOD, GOOD THINGS ARE NOT CHEAP, (Some of the best things in life are still free, as the fresh cool breeze in summer, sunlight, moonshine, blessings of Allah, unconditional love of the parents..). Just pay a little more premium, pay more and talk more, to your satisfaction. Or pay less and talk less of quality, or just listen to the voice on the other end and try to figure out, what is being said.

Definitely an issue!! It started around a few years back and I even happened to visit an operation being run from a flat at the Park towers in Islamabad. Commonly known as 'Grey Calling', it has reached an epidemic in Pakistan.
It doesn't make any difference any more if your calling directly from your cell network or through IP services such as Skype.
Technically, it is very easy to detect and block. It is really an issue, and I would also request DAWN to shed further light on this. It is a terrible inconvenience.
